Formating update, rules clarifications. 

I added chapters and reorganized the book more like a standard RPG. I had reasons for the initial unusual formatting, but I don't think they came across! 

Some of the changes:

• Clarified how tags work, how many tags a character starts with.
• Clarified how the narrator should apply disadvantage to rolls.
• Added village and city lists to help the narrator generate them.
• Updated the character sheet to clean it up visually.
• Clarified how quest generation works, added some lists of examples/choices to help create the quest.
